This policy explains how information you share through this website is collected, used, and protected. This website is operated by Eileen Chiang, an MFT trainee who provides counseling services at Heart in Balance Counseling Center under the supervision of Jeanette Shelby, LMFT, License #127144.
This policy covers this website, its contact form, and related email and phone communication. It does not cover your clinical records as a client, which are governed by Heart in Balance Counseling Center's Notice of Privacy Practices (see below).
Information I Collect
When you use the contact form, I collect the information you choose to share:
Your name
Your email address and phone number, and whether you consent to text messages
Your message, including any times that work for a call back
Like most websites, this site may also automatically collect basic technical information, such as browser type, device type, approximate location, and pages visited.
How Your Information Is Used
I use the information you share only to respond to your inquiry, schedule a consultation, and communicate with you about possible services.
Messages sent through this website are received by [me directly / Heart in Balance Counseling Center's administrative staff]. They may also pass through the technology providers that operate this website and its email system.
I do not sell, rent, or share your information for marketing purposes.
Contacting me through this website does not by itself create a therapist-client relationship.
Please Keep It Brief
The contact form is not a secure or confidential channel for clinical information. Please share only a little about what brings you to therapy, and save the details for our conversation.
The form is also not monitored for emergencies. If you are in crisis, please call or text 988 to reach the Suicide & Crisis Lifeline. Email and Phone
I use email and phone mainly for scheduling and other practical matters. Email is not guaranteed to be fully secure, so please use discretion about what personal or health information you include.
If you leave a voicemail, it may be received through Heart in Balance Counseling Center's phone system.
Cookies and Third-Party Services
This website is hosted on Squarespace and may use cookies or basic analytics to keep the site running, understand how it is used, and improve it. You can block or delete cookies through your browser settings.
This site may link to other websites, such as Open Path Collective or Heart in Balance Counseling Center. Those sites have their own privacy policies, and I am not responsible for their practices.
If You Become a Client
Counseling services are provided through Heart in Balance Counseling Center. If you begin therapy, your health information and clinical records are protected under federal and California law, including HIPAA, as described in Heart in Balance's Notice of Privacy Practices.
You will also receive informed-consent materials that explain confidentiality and its limits before we begin working together.
Security, Changes, and Questions
I take reasonable steps to protect the information you share, but no website, email, or online form can be guaranteed to be completely secure.
You may ask me to update or delete the information you sent through this website at any time.
I may update this policy from time to time. The effective date at the top shows the current version.
